name all the requirements to unlock my phone on prepaid still won't unlock.

I open the app and it is a blank white screen. It does nothing.

This app does nothing but load a white screen and when I was told I would get an unlock code it sent the code as "UNLOCK-CODE-NOT-SENT". I wouldn't recommend AT&T to my worst enemy!

you can't run the app on a locked phone how you suppose to unlock a locked phone with a app that needs a unlock phone to run app
